A helpful AI answer may still be impossible to verify. If a system cannot show where a claim originated, when it was valid, and which transformation produced the summary, it is hard to rely on that answer in an evolving project.
A citation is the beginning, not the end
A link to a source is valuable, but provenance should also identify the specific passage or record that supports the claim. Documents can be edited after an AI quotes them; a generic URL does not establish which version was consulted.
A useful evidence object identifies the source, the relevant span, retrieval time, and any validation state. For sensitive decisions, record who reviewed the interpretation.
Track transformations
If one agent summarizes a meeting and another agent summarizes that summary, a third reader may lose contact with the original evidence. This is a common source of compounding errors.
Keep summaries connected to original source records. Avoid using one generated assertion as independent confirmation of another generated assertion. Whenever possible, revisit the primary material before taking consequential action.
Handle contradictions explicitly
A product plan may say a feature is ready while a release note says it was postponed. A trustworthy system should surface that disagreement rather than silently selecting the sentence with a higher similarity score.
Phasoric's documented intelligence architecture includes evidence-aware assertions and lifecycle states, and its decision-oriented workflows are designed around explicit review. The underlying principle is broader than any product: conflicts are information, not noise.
Ask better questions of your assistant
Request an answer with cited evidence, the effective date of critical statements, and a list of unresolved conflicts. For planning tasks, ask which assumptions would invalidate the recommendation if they changed.
This format is especially useful in engineering, business operations, legal research, and financial analysis. It does not eliminate mistakes, but it makes errors easier to detect and correct.
Make freshness operational
Assign review windows to fast-changing knowledge such as prices, policies, API versions, and deployment status. A fact can have a perfect citation and still be too old for the current decision.
Build a routine that flags stale evidence, records supersession, and retains historical versions. Responsible AI memory is not merely remembering the answer; it is remembering why the answer was believed.
